Just one sentence which I think was possibly misedited: ‘For example, for every page of the statin clopidogrel published in a journal, there were 8,805 pages of an underlying CSR for the same trial.‘ Are you referring to the anti platelet clopidogrel or to a statin? Either way that looks like incredible distillation process, unless each of the 8805 pages was giant font or contained lots of repetition or demographic info.
if anyone has the patience, look at this article; it is one of what I think TTE has described as in fact as advertising copy for drug companies; rather than as these articles are flaunted; as "landmark articles' in leading medical journals!!
there were and are so many lessons from this article; as Helen comments, thousands of pages of CSR appear as a few pages; really?
the other wonderful sleight of hand in these trials is the hidden "clinical events committee" (CEC) or some such similar piece of wordsmith; go look for that in the article; what this means is a GOBSAT committee: "good old boys sat around a table":) so the heart doctor who looks after you if you have a heart attack; and participates in this trial; sending anonymised dataa about your illness; can have their clinical diagnosis of you; overruled by a committee hundreds or thousands of miles away; a committee surely appointed by and answering to the drug company: when the drug was placebo, things erred one way; if the drug was active agent, mysteriously results slanted the other way it seemed; all altered by this secretive and mysterious CEC; hmmmm; ah, don't ya just love "peer-reviewed literature
in this trial, it was reported as 360/870 deaths (41.4%) ... 41.14% of diagnoses were changed by the CEC; like politicians everywhere, they "knew best": enough for now
I hadn't seen that paper- thanks so much for sharing. Many amongst us had heard about the audacious frauds perpetrated by Big Pharma in the Vioxx scandal, thanks to people like Peter Gotzsche who have exposed in detail the rotten core of the Industry.
But something interesting happened in 2021, when Pharma were gifted a clean slate: Bad Pharma became Good Pharma; the Vioxx scandal was consigned to the history books and many erstwhile sceptics appear to have experienced a Damascene conversion. Any left behind were smeared as Anti-Science Conspiracy Theorists, whatever that means.
My immediate thought on reading your link was HOW IS THAT NOT FRAUD?!!
It is notable that the FOURIER trial was 2017 and the paper you link to was submitted in 2021. So not past history.
Of course for the Covid vaccines it wasn't just the distillation of the CSRs into landmark articles in leading medical journals - it was the further distillation of the landmark articles into a Cumming-esque 3 words slogan repeated ad nauseam: SAFE AND EFFECTIVE!

Agree completely about the status of Clopidogrel…could the confusion have arisen in their common use with statins for CAD?

This was so graphically driven home to me when I read this article a couple of years ago https://bmjopen.bmj.com/content/12/12/e060172 and dived into all the associated material;

I did a presentation on it; as one works through it, and sees the examples they gave; it was quite staggering;
